What CQC reported
Date of Assessment: 02 October 2025. Formby GP Services is a private independent consulting doctors service operated by Formby GP Ltd. The service is registered for diagnostic and screening procedures, maternity and midwifery services, family planning services, slimming clinic services and for the treatment of disease, disorder and injury. The provider offers general practice services and slimming clinics. The service provides a maximum of 12 appointments per day. The staff team currently comprises three GPs, one nurse and one practice assistant. A service is provided to both adults and children. This service is currently unrated because this is the first assessment of the service since it was registered with CQC in November 2022.This was a focused assessment which looked at the core safe and well-led key questions in response to concerns we received. This assessment did not cover all quality statements in our Single Assessment Framework which means we have not awarded an overall rating or rated the key questions assessed. We found improvements were needed to some of the systems and processes for monitoring the operation of the service to ensure safe care was provided. We found a breach of regulation in relation to governance. We have asked the provider for an action plan in response to the concerns found at this assessment. However, we found that there were systems in place to protect vulnerable people from abuse, the medicines we reviewed were appropriately managed, there was a clear learning culture and staff felt able to raise concerns. Leaders and staff had a shared vision to provide person-centred care. There was a culture of continuous improvement.
